Day Habilitation Direct Support Professional – Full-Time
8am – 4pm

SUMMARY:   To perform direct care and support of individuals with developmental and other disabilities in the Day Habilitation program.

ESSENTIAL DUTIES and/or RESPONSIBILITIES: include the following.

  1. Ensure, safeguard and advocate for individuals’ human dignity, rights and liberties.
  2. Provide meaningful activities to individuals’ as assigned each day.
  3. Provide support to Day Hab program areas to cover staff absences both scheduled and unscheduled.
  4. Support individuals with disabilities in developing and refining skills promoting health, safety and functional independence.
  5. Provide feedback to Plan Coordinators, Supervisors, and others of the Team on individuals wants, needs, skills, and achievements to better tailor Day Hab Plan’s with individuals’ Valued Outcomes
  6. Provide transfer assistance for personal care and/or other hygiene needs as assigned. Such assistance may include lifting.
  7. Provide personal care assistance to individuals as needed and as assigned.
  8. Provide opening and closing Door Duty/Transportation as assigned.
  9. Ensure and advocate for individuals’ health, safety, and independence.
  10. Assist during fire drills with individuals as assigned by the Health and Safety Coordinator.
  11. Augment current job knowledge by participating in conferences, seminars, workshops, etc., conducted both outside and within the agency.
  12. Other duties as assigned by Assistant Coordinator(s), Day Habilitation Program Manager, and/or by the Director of Services.

EDUCATION and/or EXPERIENCE: Experience/training in working with developmental disabilities preferred.  High School Diploma or equivalent required. Valid Drivers License required.

Direct Support Professional-LIFE – Full-Time
8am – 4pm

SUMMARY: To assist in the provision of community based experiences with adults who are disabled. As a member of a Day Habilitation team, maturity, responsibility, sensitivity, and the ability to work independently are essential traits.

ESSENTIAL DUTIES and/or RESPONSIBILITIES: include the following.

  1. Provide instructional activities both center-based and community-based, which will develop new skills and/or enhance existing skills related to fuller community inclusion.
  2. Provide direct, hands-on personal care required by each participant (may include lifting and transferring).
  3. Educate the general public as to the value of integrating individuals with disabilities into community life.
  4. Incorporate participant choice and decision making into all phases of planning.
  5. Participate in initiating new methods and/or ideas that will lead to more effective learning for the participant.
  6. Maintain daily progress notes for each participant as assigned.
  7. Operate program vehicles in transporting participants for off-site activities.
  8. Collaborate with Service Coordinators, VSA’s, Career Development Specialist, Day Habilitation Plan Coordinators, and the Health and Safety Coordinator as participants-needs require.
  9. Participate in Day Habilitation meetings as scheduled.
  10. Augment current job knowledge by participating in conferences, seminars, workshops, etc. conducted both within the agency and outside the agency.
  11. Conduct observations, interviews and perform other duties as assigned by the Day Hab Coordinator or Vocational Services Coordinator.

EDUCATION and/or EXPERIENCE: Experience/training in working with developmental disabilities preferred.  High School Diploma or equivalent required. Valid Drivers License required.

Contract Cleaner / Custodian (Syracuse Location)

SUMMARY: Clean customer facilities as instructed by Janitorial Crew Leader or Site Supervisor.  Duties will vary dependent on specific contracts and locations.

ESSENTIAL DUTIES and/or RESPONSIBILITIES: include the following.

  1. Follow the directions of the Crew Leader.
  2. Complete assigned task list for each specific assignment which may include:
      • Checking all restrooms in assigned area
      • Cleaning restrooms and re-supplying as necessary.
      • Sweeping and mopping of all restroom floors.
      • Cleaning facility windows.
      • Dusting all desk tops, cabinets, ledges, partitions, furniture etc.
      • Cleaning floors as required, using floor signs and all appropriate safety procedures.
      • Emptying waste receptacles and replacing liners as needed.
      • Removal of all recycling material and placing in appropriate bins.
  3. Properly maintain equipment.
  4. Report equipment failures promptly to Supervisor or Crew Leader.
  5. Report any problems, complaints or incidences promptly to the Supervisor or Crew Leader.
  6. Inspect area for Cleanliness and ensure no cleaning devices (chemicals containers, equipment, rags, etc.) are left behind.
  7. Overtime as required.
  8. Other duties as assigned by management.

SAFETY RESPONSIBILITIES:

  1. Wear personal safety protection appropriate to the area that work is being completed in.
  2. Be knowledgeable in bloodborne pathogens and apply proper safety precautions.
  3. Be knowledgeable about cleaning products and chemicals and use in accordance with prescribed recommendations.
  4. Use ergonomically correct posture when completing repetitious work or lifting pails, etc.

EDUCATION and/or EXPERIENCE: High School Diploma or equivalent education. A minimum of one (1) year janitorial experience. Must have ability to work productively when unsupervised.
